GUYS I MIGHT'VE FOUND A FIX FOR NOW
The problem MIGHT be how the windows power plan affects the game, maybe that's why some people have the problem and some dont depending on their plan.
try putting your maximum processor usage in the power plan to 99%, you can do it via lasso too and make a custom plan that activates whenever you launch the game. I tried it for hours and I havent had a single freeze or crash. My PC was freezing all the time prior to this (possible fix).
Tried this method and it didn't work for me. Hard crash after about 40 minutes of gameplay when loading into a new zone.

This problem has happened to me about 10 times since I started the game. Each time I had to turn off PC.
CPU - 7800x3d
I hope this problem will be solved as soon as possible.

This issue happens to me only when loading between zones. Other than that, the game has never crashed. It randomly started after about 5 hours into the game with zero issues. Maybe it was an update or hotfix. I can't be sure. This is likely old information, but here's what consistently works for me.
Turning off Multi-threading right before teleporting or waypoint fast traveling (anything that requires the loading screen with the two gears).
After loading in, I turn multi-threading back on and continue playing at high FPS.
I know it's tedious, but it always works for me.

Imagine having a beast of a pc (we have the same specs) and having the need to disable cores. This guys are out of their minds.

I have the same setup, the only difference I have 64gb ram. I stopped having hard crashes. I still crash to desktop, sometimes I get a crash to desktop with po up window missing textures.
I updated my bios and left everything stock except ram xmp. Re downloaded AMD chipset drivers. No more hard crashes, still getting crashes to desktop.
In my previous bios setup I had the motherboard TdP set to max 105 watts. After I updated new bios I left it stock. Playing multithreaded no hard crashes.

To all the people that have new x870e and x870 motherboards. In bios leave your motherboard TdP stock and don’t use gaming mode.

This is what has been working for me:
Edit the game ini to use the Vulkan renderer.
On next boot, change the renderer to DX12 and let it 'settle', you can tell this by the graph in the top right corner.
Change the renderer back to Vulkan.
Each time I but the game I change it to DX12 then back to Vulkan and I have had success every time since then.

I thought the servers were overloaded, but I'm stuck with the same issues people report here.
After reaching Act 2 and killing the first boss there, teleporting to the Clearfell Encampment crashes the game entirely every single time.
I suspect the issue started since Windows updated to 24H2.
System:
RX 7900 XTX
7800X3D
32 GB RAM
Hmm, I feel a bit dumb, clearing the %TEMP% folder and rebooting my PC fixed the issue.

If it helps to someone why not?
Did you try to change the process priority?

It sucks, but I don’t lose out on performance because I only need to do it before teleporting. Like I said, it’s tedious, but allows me to play with no issues.

That sucks. I wasn’t having any crashes to desktop, just the hard lock between loading screens. I believe I’m using an x670e motherboard.
