Yeah I tried dropping all my simulacrums and ultimatums restarting and killing the boss but still nada. Also ran a monster level 68 ultimatum with 7 waves cleared that got nothing. Ran a 56 2 floor simulacrum and nothing. When the djinn it dies u can actually see the item drop but nothing is actually on the body.

same here.
i could open a 2 level one in the normal act 2 shrine, but upon entering i get an "out of sync" error and get booted to the main menu. hope this gets fixed soon, pretty gamebreaking, appereantly you cant advance to maps without finishing it first. Same here :( Didn't get the quest coin for cruel act 2 trial after killing Balbala, so my quest is stuck on Travel to Traitor's Passage.
