Poet's Pen not casting 2nd attack when getting fractionally close to 4.00 APS

20% increased cooldown recovery speed should give me 20% more casts, up to 4.8 casts per second. My current APS is roughly ~8% lower than that, which should be more than enough to account for the difference in APS between my weapons.
Even assuming the worst case which is caused by a cooldown rounding issue - 250ms / 1.2 = 208.333ms which is rounded to the character sheet value?? of 210ms that still comes out to 4.76 casts per second which far exceeds the APS of my weapons, including the uneven attack speed.

TY for linking your belt. Wasn't aware that mod existed. Sure, looks like your issue fits well in this bug report. My mistake on the oversight. Hope we can get some attention to this issue from GGG soon.
I can't say exactly what aps but when I was at 4.0+ with the frenzy charges it didn,t cast volatile dead on every attack/cast with Poet's pen, and in hideout no issues (without the frenzy charges at 3.34 aps)
Can confirm I have this issue. Slowly dropped my faster attacks gem level until I could manage to minimize 'poof' attacks. When I was close to 4APS had nearly 1 in every 3 attacks 'poof'.
Character sheet reads 0.26s attack time, using 2x 11% AS poets pens. It appears to be around every 9th attack a proc is missed. I accept this as a reasonable margin of error as it's not often I sit still long enough to smack 9 frenzies into a single target.

When you dual wield the wands and use an attack such as frenzy, that alternates between 2 weapons poet's pen fires spells from BOTH hands.
Pen number 1, should fire all spells in that hand on attack when the right hand is used and pen number 2 should fire when the attack alternates to the left hand. This is however not the case. it uses the spells in both hands on both attacks and only alternates between them, if there is 2 spells in one or more of the weapons.
attack one: Unearth + Volatile dead
Attack two: Bodyswap + Volatile Dead
Meaning that if you have more than 4 aps, your Volatile dead will cast slower as it is still on cooldown once the second attack procs. yet unearth and bodyswap will not, since they alternate between the two.
If it worked with alternate attacks with same setup:
Attack one: Unearth
Attack two: Volatile dead
Attack Three: Bodyswap
Or
Attack one: Unearth + Body swap
Attack two: Volatile dead
