Same here, can't complete the quest anymore after having fought Kitava.

This is dumb, I can't complete the 'Kill these unique monsters encounters' achievement on my level 93 char. I full cleared ruined square 4 times and never saw Utula.
Based on reading through this thread, it seems the only way to fix it is to create a new character and consciously create a new zone until utula spawns because once you get past a5 without killing utula, there is no way to kill him after that because he won't spawn. |

The workaround would be to join a party with someone that has not yet done kitava, and have them create the instance, so he spawns, then have them TP you into their instance...

Is this defined still as a defect? I've cleared the square 2 times now and haven't seem Utula. I'm currently mapping and went back to finish off any incomplete quests.

After posting the above, I spawned a new map for the square and lo and behold, there was Utula - 3rd times a charm. So he's a random spawn, no issues here, good luck.

5 times cleared and no sign of Utula... Any chance to have a clue of what's going on ?

I'm a bit confused by the reports in this thread. Yes, you can technically skip the Utula quest and just go kill Kitava right away, but it shouldn't break anything.
It doesn't matter if you have killed Kitava or not. It doesn't matter if you have killed Utula or not. It doesn't matter what level you are. It doesn't matter what NPCs you have talked to. It doesn't matter if you have the quest or not. It doesn't matter what state your character in. Utula ALWAYS spawns in the Ruined Square. Always. Just like every other boss, he always spawns. He even spawns in roughly the same place (opposite side of the square from the exit to Cathedral Rooftops). He has a little park with a giant bloody X made from bodies and stuff. The only way I can think of where you could miss him is if you joined a party and entered an old instance where someone else had already killed him while you weren't there. (all bosses work this way). I've tried to repro this - just in case - dozens and dozens of times, but every time Utula spawns as expected. Not sure what else I can do at this point. Code warrior
