No to all three, he only has to be in the area for all the bonuses to apply. Some people abuse this by parking level 1 mule accounts at the entrance.

As long as he is still in the map, yes.

I'm not sure if dead party members still leech experience away from other people. Since they can't gain any experience, they shouldn't, but who knows...
Just to add to that, experience is only shared with other players who are near you (roughly two screens). This most likely also means that you don't share with dead characters (since dead characters don't gain XP while dead). Monsters are still made harder by players elsewhere on the level.

Any skill that has an effect with a duration is affected. Damage over time effects have their duration increased, meaning they will do more damage over the course of their full duration. They will still do the same amount of damage per second though.

What about Double Strike?

For the purposes of stun, Dual Strike and Cleave count as one hit, Double Strike counts as two.

not sure if this has been asked yet, but in relation to Resolute Technique, I presume it overrides an opponent's evasion rating, but does it also override an opponent's shield block chance?

No block and evasion are differents tests.
in other words, "always hits" can also be "blocked" have i gotten that right?

Say I have iron reflexes. All of my evasion is of course converted to armour but is that before or after bonuses to evasion from passive nodes?
That's about the umpteenth time someone asks that...

When you have Iron Reflexes, all your Evasion basically turns into Evasion Converted To Armor. This value is simultaneously affected by % increases to Evasion as well as % increases to Armor.
