I looked at your end game passive skill and I failed to see more than 123% Life from passives.

Thanks for this, I actually did have a calculation error there. The value is +243% with flat +50 HP, not +253%. Updated to v1.55.

Have you thought about having 2 infernal blows, 1 for damage and 1 linked to culling strike, item quantity and item rarity? If you can focus down the champ first then it should be pretty effective.
