Don't run low life in Cruel, the enemies are pretty weak and the little damage increase you get at that low level is not worth giving up 65%+ of your life.
The biggest problem in this case is your gear, Tabula Rasa has close to no defensive stats.
Get an Icetomb for the major part of Act3, and equip Zahndethus Cassock for the Sewers and when facing Alal the Terrifying (second stage of Dominus fight). Stone of Lazhwar is also mandatory for both Piety fights and everything from Battlefront onwards. Other than that, i would also suggest not to level as a full MF'er but rather switch to that after you finished Act 3 Merciless.

which is best? increased AOE or concetrated effect ?

Conc Effect for single targets, Increased AoE for taking out packs. I usually have both gems with me, f.e. in maps Increased AoE for most of the part and then switching to Conc Effect for the boss.

Ok, im in act 3 merc and im dying to nearly everywhere except fellshrine now. Resists are capped but i just dont know what the fuck to do.

How much total ES and Armour do you have?

I would spec out of Discipline & Training and Holy Strength, also taking the damage nodes in templar area is not that useful yet, better respec back to Elementalist and spend the 9 nodes you've gained to reach Ghost Reaver and add Life Leech to Tempest Shield (and maybe Molten Shell). You will need that in Merciless until you have an Aegis, have written about that in FAQ section.

After that, you should get your Aura nodes as they will add additional survivability.

Edit: Gear wise, idk what's your budget is neither can i see the stats of your rares (you can link items directly through forum). Icetomb gets a bit too weak for Act 3 Merc, you can find some decent 5l Ar/ES chests for 2-3 ex.

Added a little MF Log of 100 Lunaris 3 Runs, you can find it in the "Media" Section.

Hey I first wanted to say thank you for the build. It definitely is very interesting and I stuck with it through some of the slowest leveling I've ever been through because I wanted to see it come to fruition.

That being said, I have a few questions:

1. First, around what lvl did you start to feel more tanky? I just hit 62 and finally got my Aegis on, but I still seem to be dying to packs in A3 Merci. In essence, I see how the build is supposed to work, but I seem to be taking large hits so my shield goes down too fast.

Here is the equipment that I am currently using (don't mind the gems, just throwing things around atm):

Hey, glad you like it :)

1. To be honest i always felt very tanky, it's a matter of when to use your Granite Flasks and Stone of Lazhwar. If you see huge, or many physical hits incoming, pop your Granite Flask. If you see many/only spellcasters around, equip Stone of Lazhwar and they will barely scratch you.

You're missing lots of Armour on your gear. I can see why you went with a pure ES chest and it can work, but you gotta need lots of Armour from other gear, which you currently kinda lack. If you can get the Fire Res from an other chest, i think i'd have a huge improvement for you:

Aurseize aren't an optimal choice either. Good resistance Ar/Es gloves are pretty cheap and will give you more resistances on top. Last but not least the belt - you need a Heavy or Chain belt with +Armour and +ES roll. Those again can be obtained very cheap, my tip is check gear at poe.xyz.is first (choose buyout only), put together a constellation that maxes all your resistances and then buy it.

2. Clarity and Discipline. Once you picked up Pain Attunement, run the third (Determination) on life.

3. Lunaris. Safest spot for this build to level even without having Shavronne's Wrappings.
