We are looking into this, and have had it happen locally, so we'll try and figure out the real cause!

when using a 20% level 18 LW and a 20% level 19 faster casting.. along with about 40% faster casting from gear.. it happens once every 3-5 warps for me..without using a speedflask. I did notice this before the recent patch as well but due to the slower speed I did not use it quite as much as I do now.. it's gotten very apparent.

If the issue is happening locally for them, then I highly doubt it's a desync issue. I suspect that the issue isn't localized to just lightning warp. There are a number of skills that fail to respond 100% of the time when attempting to activate them. Perhaps tracking this one down will solve a much larger issue.

IT HAPPENS! we must fix it ^_^ I play a leapslam hero, and I know the 'thing' that happens. It doesnt 'happen' as often as lightning warp 'happens'

Same bug for me, i play a marauder based on lightning warp and i hope they will fix that.

Well after the buff to lightning warp, GGG made lightning warp a viable spell finally. Kinda like the spell version of leap slam. I think they will fix the bug once they nail it down.
