About Beyond... "Monster kills have a chance to cause small portals to appear"

Does that mean that any little white monster from first beach onward can trigger boss fights if you kill too many too fast? I previously thought it was just specific dimensional monsters that caused the portals, but now I wonder...

dont bother with him , he clearly has no idea how much damage per hit is "normal" now , crit dagger is the best way definetely but , elemental buzzsaw builds can also work with it reaaally good , 12+ attacks per second with huge amounts of elemental damage will melt mobs with a simple viper strike blood magic(maybe you wont even need to use it since lvl 1 viper strike is needed) wed multistrike melee splash (faster attacks later , just like a buzzsaw build) + lets not forget that with a simple investion of 3 nodes in the shadow area ( you will go there for either crit dagger or buzzsaw) you will get 45% increased dot damage which is calculated after everything.

You said not everybody goes crit dagger and here you have a superb (imo) elemental version , dont forget 500+ dps two handed swords which will make a not bad at all RT version of viper strike imo + now that it is worth it to scale attack dmg with viper strike you can benefit from leech and then phys leech on gear so you can sustain yourself , viper strike will become FOTM and i guarantee it , naysayers just barely understand game mechanics..

Right. Any monsters can do it. You will know it's coming though.

So - do summoned minions, fractured mobs contribute to Rampage count/Beyond portals?
