Hi got the same bug today.
the item droped without the clasic green hue as other quest items, and being curious, i deleted the item in town, to see if the game hadn´t registered when the item droped. The game let me delete the item no problem.
then i tied killing balbala again and it didn´t droped the quest item, so im in a lot of trouble i think

plz fix bug.

Same here for me and my friend no luck progressing past Search Traitor's Passage.

same problem here too in cruel, quest lock to ascent to power.

In the map, location of trial of sekhemas is level 22 (in cruel difficulty)

So i found the solution, after asking around. After last update, tooltip on my trial items with 2 floors says, that you will get only the option to "choose" your ascendancy and not the points. You need Barja coin with 3+ trials (floors) in tooltip - to get the 3rd ascension (5th and 6th points) and those drop usually much later in atlas maps.
The same goes for the chaos ultimatum items, you need 5+ floors ultimatum items to get 3rd ascension points.
And you can just google "poe 2 3rd Ascension" and there are YouTube videos to prove that.

I killed Balbala in cruel and there was no progression to the quest even though she dropped a Dijin coin. Killed her several times after that and she stopped dropping the coin and the questline was still stuck having me search the traitors passage. I dropped the coin in town and destroyed it but now after a couple more times of killing her she still won't drop the coin. So it seems like I can't do anything with my 3rd ascendancy.
