In general you just have to follow the guide, way too much is being ignored but get life on all gear and correct flasks immediately.
The help section in the bottom of the guide should answer all your questions.
Most of the questions are about their next items to upgrade, your guide does have item options along with the prices from last league, but they don't really know how big an upgrade an item would be and if it's worth the cost. The only item which mentions a priority is paradoxica which isn't very cheap. People just want the biggest bang for their buck. Sure it will always depends on what they currently have, but a general priority list could be nice. Here is an example, maybe something like:
#1 Abyssus (cheap, biggest damage upgrade possible, cost survivability but worth it, replace for one with enchant later)
#2 mana leech jewel
#3 6L with max life and resist
#4 Blind + quality Paradoxica (when you feel like saving up for this, go for it I don't recommend a cheap 2-3ex ones cause the good ones are like 8ex)
#5 %life on block shield (if you have survivability issues and dont have this yet, get it)
#6 Thread of hope #1 (left side of tree)
#7 Vulnerability curse on corrupt gloves or crusader ring (try to find a good mix that cover the curse prioritising steel ring if possible can use deafening essence to get good rings if resists are already covered too)
#8 Ryslatha's coil (another expensive item, but really huge damage increase)

GEAR
The prices in this thread are 1 month into Heist league with Exalted Orb being 70 Chaos.
The prices are not set in stone and depend on how popular the build or melee is.
Weapon & Shield

Don't spend money on getting high phys% damage. 80% vs 100% is only 3% damage difference but 10 EX more expensive.
Blind is FAR better than Impale and gives you much better survivability in T19 100% Delirious Maps - Impale is only 1% more damage.
If Paradoxicas are expensive, use Path of Building and compare what a rare foil would get you for the same money.
30 chaos.
Get it as soon as possible.
Even a badly rolled Paradoxica is better than a good Ahn's.
5 Chaos.
Set your budget between 5 and 10 chaos - get the one which has the most pDPS.
5 Exalted.
Must have life % gained when block and life.
1 Exalted.
I'm using this to speedrun AW8 Sirus only. This shield gives me 11% more damage which is a lot! Global phys is a standard mod and 1% increased Damage per 1% Chance to Block Attack Damage is an Elder / Crusader mod.

Your goal.
You want explosion + frenzy on hit most of all - both are very important.
25% increased effect of Non-Curse Auras from your Skills is also really strong - 10+% more damage!

1 Divine / 8 Chaos.
Don't buy a Tabula if you're high enough level to wear a rare 6L.
Tabula even in league start is the same price as a rare 6L except it has no stats.
Helmet, Gloves & Boots

You want below 42% increased physical damage taken and at least 110 critical multiplier.
Don't bother with helmet enchants unless you're willing to divine it to 40% and 120+%. If you do bother - Blood & Sand effect( best), Reave Radius, BF damage, Ancestral Warchief effect, Dread Banner effect are worth it.
Considering how tanky the build itself is, I don't suggest any other helmet.
If you die too much, remove some damage nodes and take some more life.
For XP ROTAs equip any rare helmet with some HP.
If you have Vulnerability on rings, you have free reign over your gloves selection - I just find it considerably cheaper to get it as a corruption on gloves.
40 Chaos.
These with Vulnerability are actually best in slot due to how much we use Vaal skills. Rare gloves with Vulnerability curse would work if you don't have a curse on rings.
1 Chaos.
Should be always used before the explosion chest.
Vulnerability version costs 1-2 EX so Enfeeble is great budget option!
40 Chaos.
They're common and cheap as the combination is not often used.
You want life + increased duration + fortify to have a pseudo 6-link for your Vaal Double Strike.
It's also possible to get Hunter's Tailwind.
Belt, Rings & Amulet

The MAXIMUM stat is far more significant than the MINIMUM - try to buy or divine MAXIMUM to as high as you can.
1 Chaos - 1 Exalted.
Get as much life and resistances as your budget allows, this is a good belt to avoid high expenses on resistances in league start.
1 Chaos.
Cheap alternative if you don't need to cap resistances and just want some more damage.
10 Exalted - seems like it's very expensive in Ritual.
It's absolutely and totally worth it.
10 Chaos.
You want life & physical damage the most.
3 Exalted.
You want life + physical damage and then as much resistances as possible.
5 Exalted.
15%+ Global Multiplier is highly suggested - can be crafted only with Elder influence or essences! 22% on both rings is a total of 16% MORE damage!

You want 2 additional impale hits.
One of the strongest Watcher mods - around 20% more damage, worth it.
Always divine the life to 7% as the impale mod cannot change.

Corrupted Blood is almost mandatory, playing without it is a lot more stressful when doing the hardest content in game as Corrupting Blood is everywhere.
Rare jewels should have life % most of all.
Global multis and weapon specific attack speeds are the best.

Need mana leech from somewhere!

If the gem's socket location has no inherent bonuses, then it's chosen based on where you're more likely to get the colours.

You can buy these with correct levels from their respective act vendors.
Vaal Reave - Body
Make sure your Vaal Reave is level 20 or 21 as it provides +1 radius - this should be one of your earliest purchases.
It's best to get 4 stacks and then use your Vaal Reave to push it to 8 stacks.
Blade Flurry is not mandatory at all but it is useful if you're planning to kill Sirus & Shaper often.
It's most effective to hold until you have 6 stacks and then release.
Getting 20 quality early on Infused Channelling is a very good idea!
Blade Flurry - Impale - Fortify - Melee Physical Damage - Brutality - Infused Channelling OR Close Combat

Vaal Double Strike - Boots
This becomes a pseudo 6-link if you have the proper boots.
As it has 2 charges, it's wise to plan them ahead and use once every phase if the boss has multiple phases.
Precision, Herald of Purity & Skitterbots - Gloves
It's best to put them in gloves because the colours are hard to roll in the helmet.
The lower your Enlighten level is, the lower your Precision gem can be.
You want to have at least 100 mana free.
Precision - Herald of Purity - Summon Skitterbots - Enlighten
Whirling Blades, CWDT Immortal Call - Weapon
CWDT and Immortal Call have to be level 9 & level 11.
You can buy CWDT & Immortal Call with proper levels from Act 4 if you're level 55+. It's good to get quality asap on Whirling Blades to move faster.
Cast When Damage Taken Lv. 9 - Immortal Call Lv. 11 - Whirling Blades
You should spawn Ancestral Protector first and then Vaal Warchief, not the normal Warchief.
These two buff our damage by almost 90% + Culling Strike - using them correctly is the key to instaphasing all bosses!
It's highly suggested to change Culling Strike to Divergent Riposte if you're plain mapping and not strictly doing bossing content.
Levelling this build is very easy as there are a lot of 2-hander nodes nearby and two-handed weapons are dirt cheap even on the first day of the league.
Two-handers are also extra strong with Warcries early on.
4 links and two 1C physical weapon upgrades is all you need to complete the acts without any struggles.

After levelling up with two-handers, you should regret all points which the build doesn't use.
Some more tips:
After finishing acts, type /passives to see which quest passive points are you missing.
Do the sidequests immediately.
Do each lab before each Kitava fight - the first lab is extra good for levelling.
Filter out Wisdom Scroll already in the start - Whetstones give 4 Wisdoms when sold.
Crafting when starting the league during levelling is a good idea!
You can buy level requirement 50+ gems from act 1 - 5 after completing the acts.
Buying items & mapping

Use "COUNT" if multiple affixes fit you.
Useful for jewels.
Use "NOT" if there's a mod which you do not want.
Useful for Paradoxica.
Use ~ in front of the affix which you're searching if you're unsure what the affix exactly was named.
VS
Hard map mods
The build can do all map mods except physical reflect, even cannot leech ones can be done but you might want to avoid these map mods for sure if you're doing T19 100% Delirious maps.
Regarding maps there's no need to rush T16+ maps if you don't have enough Atlas completion as you'll eventually have to grind it out anyways and the experience gain in yellow maps is higher than in T16s if you're below 90.
GAMEPLAY
Important gameplay mechanics

1. It's important to have your Vaal souls ready for the bosses in maps to gun them down fast.
2. It's important to use your Vaal souls as consecutive as possible as the soul gain restriction applies to all skills.
3. There is no use in using two Vaal Warchiefs, one is enough and it's used for DPS boost.
4. Vaal Reave is used for range increase and when OOM, if you use it carelessly you might not have enough souls for other skills.
5. You should always keep your totems up at bosses as you gain ~ 40% more damage from Protector and ~ 70% more with Vaal Warchief added to it.
Maximizing your damage & defences

To set up the perfect boss killing settings you have to do the following:
0. Make sure you have Blood Stance
1. Ancestral Protector
2. Vaal Ancestral Warchief 1.5 seconds before phase start
5. Use VDS clones 1 second before phase start
3. If you have Blade Flurry, start channelling it
4. As soon as the boss is vulnerable - use all flasks and release Blade Flurry
5. After the soul gain prevention use Soul Ripper and repeat.
You want to use VDS clones only when you're certain that they're able to do their fullest of damage.
Remember that souls are allocated randomly to where there are souls missing. This means that as an example it's sometimes better to not use your Vaal Warchief in order to fill up your VDS up with Soul Rippers.
You may use two Soul Rippers if you're against any other boss except Sirus.
It's also a good idea to spam your Soul Ripper if you have them with low charges just to get the 10% Dodge & Spell Dodge + phasing for maps or the Simulacrum.
Since we use Tukohama & Steadfast - after not moving for 4 seconds, you have an additional 10 physical damage reduction which is equivalent to 3 Endurance charges.
It's most preferred to have 6.5K+ life if you're levelling.
If you're not levelling, it's important to reach 5.5K+ life in order to have decent survivability with some deaths once in a while.
It's important to not ignore your Pantheon - the bonuses provided by both of the Gods are very strong! Eater of Souls allows you to ignore critical modifiers on maps completely.
Since the build is tanky enough, chaos damage can be ignored but it's the biggest weakness. It's a problem mostly for the Hunter Conqueror if you don't instakill him. If you have open suffixes, taking chaos resistance is an okay idea.
If you're doing T19 delirious or in general really hard maps, you should try to constantly keep up a Vaal Warchief to have 45% more damage and Onslaught if you have Vaal Caress gloves.

So it looks quite good so far, however I struggle espacially in Ultiamtums getting wracked and in higher Maps like T9++.
I know I still lag Gear and stuff so I jsut want to ask what you think so far:
Whats the Next go to upgrade? I still need the Thread of hope Jewel to allocate the passives not linked to the tree. Is that the next thing to do?

flasks. use the front page ones especially Lion's Roar unique big dmg boost
you only need 1 health flask for corrupted blood immunity for now early on.
those others can be replaced to boost ur defenses/dmg
OK bought all the Flasks now and another Life Phys DMG Ring:

According to the other comment "I ignored too much", I am not sure, as a casual I dont have 2 Ex to spend, and I focused on life on all Gear as much as I could afford, perhaps its just done then for me, and I try another build fair enough. :)
Oh and I get Immune to corrupted blood from an Jewel I found.

I'm sorry if the question has already been asked, but I can't find any answer:
How many totems can we invoke at the same time ? I saw on some of the showcase videos that you have 2 of them, so I guess it was from the "Panopticon" node on the tree. Unfortunately, after taking it, I still can't invoke the 2 totems at the same time. Did I miss something ?

You use an Ancestral Protector totem and a VAAL Ancestral Warchief totem. You can have both going at one time. Panopticon not necessary.
You can't use Warchief and Protector at same time. You can use Vaal Warchief and Protector. If you look at the Vaal Warchief text you can see it has a "+1 maximum summoned totems tag" allowing you to temporarily go to 2 when you use it.
