It's hard to say 100% what the issue is based on what you're saying here, but I think the issue is you're running out of mana.

Your hit chance is 89% so I don't think you're missing attacks. Your mana regen is really low though. No boot enchant and you're not using arcane surge buff. Minotaur has a lot of AoE damage though so MoM and cyclone are probably taking all your mana hence why you cant attack and leech your life back.

GGG nerfed inquisitor's mana regen into the ground last patch though so this build with inquisitor is now dead. It can still run maps easily, but boss fights are a pain in the ass now.

Is it viable now for Heirophant + non-legacy indigon? And if going Heiro would it be good to combo discharge with storm brand?

It will be tanky, it will do all content (maybe except Uber Elder) but just dont expect cosmic DPS anymore.
