Crashing a lot Info includes Dxdiag + crash errors

I have managed to get my game working by doing the following:

Turn off overclocking.
Lower all settings (except resolution) to minimum.
Run in windowed or borderless window mode.

I still crash once every hour or two.
I have to re-install the game every 3 days or so.

I know very little about how programs operate but i heard it mentioned a few times that it is a memory leak. Apparently this will build up overtime and cause crashing.

Yeah apparently with the very new cards it's super optimized . I bought myself a 1050 ti 4gigs and never had an issue once, well except lag spikes a few times but that was on GGG and they fixed that

I have a I7 7700k, GTX 980 TI SC+, 16gb 2133 Mhz RAM, 1 TB SSD on Windows 10 pro and I get black screens constantly. I normally play hardcore but literally cant cause I constantly die due to black screen crashes in PoE. I Have tried everything and cant seem to fix it which leads me to believe it is something on PoE's end. I can also run CSGO, PUBG, Destiny 2, H1Z1, WoW and have had no issues at all with those games but I crash playing PoE.
