The shift is most probably caused by Dock, or system menu. Also, the 4.0.1 should not have offset issue (at least not in "fullscreen" mode)). The only things which should result in offsets are resizing game window by dragging it by border, or any custom application which modifies/adds dock/system menu behaviour.
In another words, I never had such issue, except when resizing.

How do i remove or uninstall this? I guess there is more files than just the Path of Exile in my app folder?

Move app to trash
Config files are stored in Documents/My Games/Path Of exile

Aside from massive zone load times and the occasional crash, 4.0 and 4.01 run great on my machine. (OSX 10.8.2, MBP 2.4GHz i7, 16GB DDR3 ram)
If you're getting the video memory error crash (I was), open wineskin (show package contents of wrapper) and add the EXE tag "gc-1" without quotes.

For example, I can run 4.0.1 without crash few hours on 10.8. It pretty much depend on other things as well.
For example, the game tends to crash/freeze on windows too. I know that wine is not perfect, and may freeze by itself, but when the game is not perfect on windows, it is really hard to distinguish between game and port fault.
That is also a reason why I need the logs. If there is an crash dump on the end of the logs, I can determine if it was caused by wine, or game.
You can put logs there in spoiler tags (above edit box), or upload for example to pastebin service. also about 200-300 last line will be enough. How to get logs are specified in FAQ at 1st post.

You can save 5GB of downloading by copying the content.ggpk file from old wrapper. (1st post -> FAQ > How to move game file from old wrapper). Or you can let it download it again.

The -gc 1 (you have typo there :), is responsible for massive loading times, try to set it to 2.
The -gc stands for garbage collector. Basically it tells how many zones (?) should be kept in memory. Setting it to 1 will always result in loading whole area again, thus massive loading times.
