Death Penalties

@Albain
you might do that in Hardcore Leagues etc. but not in a softcore league as it would most probably scare players away
IGN - SunChaos, Syldra
Hardcore and Softcore should be divided. Incorporating a system that is to similar wouldn't really make a difference to many players.

Softcore is to keep long time fans of the genre and attract new fans in that order so they can experience the full game without giving up.

Hardcore is NOT about being "ELITE" but rather being humbled by your many failings and rising above them.

Dying in softcore means you should lose part of the level you had gained (have a tombstone like in titan quest or use something similar so you can regain part of the EXP and RENOWN that you had lost, but NEVER ALL OF IT.

Dying in Hardcore should allow you to make decisions about the life you've left behind, to impact the game (like in Demon's Souls) maybe leave messages, or leave a strong presence that either scares or attracts monsters to it (so you can reclaim your loot with a new character unless someone beats you to it).
I love virtual brutality so save it for the Mobs...
GGG has already discussed the system. Basically, after a hardcore character dies, you can no longer play him/her in a hardcore league. Instead, you're transferred to a regular league. The items will not be available to hardcore characters; however, they will transfer with the original character to the regular league. So, if someone wants that intensity, they can play hardcore league. I like the system. Let's say you've spent months leveling your character and you die... well, that sucks since you're not in hardcore league anymore, but at least you did not waste all that time and can just continue in the regular league. Less prestige, but you can still play the game.

The following are examples of leagues that we are working on designing. Feedback and ideas are greatly appreciated - we're very keen to hear what you think on the forums.
Hardcore

Typically, “hardcore mode” in action RPGs involves permadeath. A character killed in this mode cannot be accessed any more. In Path of Exile, slain hardcore characters revert to the parent non-hardcore league. This system encourages non-hardcore players to try the hardcore game mode, while still permanently removing hardcore characters from the economy when they die.
Ironman

In Ironman, players are unable to trade with vendors or refill their flasks in town. Mana regeneration is disabled by default. Players are encouraged to band together and manage their resources in order to survive.
Cut-throat

The cut-throat leagues appeal to the niche of players who demand the most hardcore gaming experience possible. In this mode, all world areas are public, with full PvP enabled by default. Slain characters drop all their items upon death. This game mode can also be combined with Hardcore to create an even more unforgiving world.
Attrition

An attrition league is a short duration league that starts with a fixed number of players and eliminates the player with the lowest amount of experience at periodic intervals. For example, a 24 hour attrition league might start with 100 players and eliminate one player per 12 minutes after a four hour initial leveling period. Players would be able to see how close to elimination they are on an on-screen ladder display.

I think that when you die you lose your weapons but keep everything else and you go to heaven and talk to some holy god or something and you pay him/her a fee if you can't afford it then you have to do a job for him. Hope you like it

I think it would be kinda cool to see a tag on characters that keeps track of deaths. Something that other players can see on eachother. It could be like a badge of glory if you have very few deaths or a badge of dishonor. Other players would get a feel for another players' skill level very quickly. Possibly even put a ranking of highest levels suffering least amount of deaths type of ladder? It would make dying be taken a lot more seriously and add a degree of competition. What do you think?

This is effectively a "corpse run" which pretty much everybody hates. I doubt this would be well received.

It would only be okay for a special League... of jerks... who deserve to get what's coming... a huge mob when you hit 200/1 kill to death ratio! Hell yeah! you are fighting each other for the glory of a creature you don't know exists and it owns all that try... the only way to kill it is to join forces but there are no teams and everyone's attacks hurt all in their path! Yeah I still don't like it but if there were monsters that were summoned upon a very high KTDR that gave great exp/loot/renown to all nearby upon death... that may change my mind.

Whatever if there is still permament exp loss.
If there isn't permament exp loss then it is bad.
