You've narrowed it down to the helm, which is correct, but it's not the chrome wolf. It's got new 2D art, by the same artist who did the avatar.
What I can tell you is it's themed around one of the lore characters. Gameplay & Level Design

hmmm two lore characters that don't have unique helms that might be it are Daresso and Marcus Lioneye. We know that at least Daresso is on Act 4.
