1. Not sure if it's been mentioned, but the grate is off it's hinges...perhaps the entrance to a dungeon. Not sure if it already exists in the current game.

2. Could the guy with the weapon be a Follower that helps you? Probably already guessed :)

I suppose that blood thing is a portal to a new area, and that guy is the guide
random quests with rewards.. blood test--time limited if fail will be punished..(unpredicted punishment)

maybe that guy is a new random npc" you can trade some limited amount of items for a low price
or a npc who sells rare or unique items for a limited time
after you complete the quest

There is something in his pocket(maybe the bag on his back)!!!!!!!!!!

Haven't read all the comments, so I don't know if it was mentioned before, but I believe the expansion is Karui themed. Maybe a new Karui invasion of Wraeclast, although there would have to be a good reason to invade a cursed land, where you help them out, or oppose them (it's a stretch but who knows >.>). Or maybe just focused on Karui stuff. It would be interesting.

Just my thoughts so far.

I'm expecting more of Kaom and Hyrri in this mini-expansion. （＾∇＾）
