PS5 experience and issues - can't ascend / map resets and others

Hi, after 150+ hours of gametime, the amount of annoying issues is slowly accumulating, so I wanted to share some in hope maybe I get some answers or at least help you to recognize the issues.

Discovered Map resets
-repro rate: 100%
-repro steps: In campaign if you die or teleport to sell items and immediately return, discovered map resets

Cannot get more ascendancy points
-Cruel mode: trying to get third set of ascendancy points is impossible. All baryas and ultimatums claim I won't get additional points. Quest is stuck in "Ascent to power - search traitor's passage" sequence. Some reddit threads offered amount of solutions (teleport there and here, run it again, both type of trials..), nothing helps. Teleporting to trial of Sekhemas location repeatadly shows Quest completed but can't get from the lock
-being locked from important part of the game is bad

Items being dropped on floor instead being transferred to the chest when triangle is pressed
-100% repro rate in locations like trial of Sekhemas

Swapping weapons disables spirit usage
-playing monk, witch - swapping weapons is bugging spirit, need to disable and enable spirit usage everytime for it to work properly. Sometimes it bugs in a way when I swap weapon sets, it keeps incrementing visual representation of amount of skeletons too (tho it would be fun (and gamebreaking ik) to have 10000 skeletons)


Visual flickering and artifacts
-there is a lot of dark foliage and background that keeps flickering and causing epilepsy

Bosses
-Viper Napuatzi if pushed/knocked during specific animation can get stuck in her comrades
-Titan Grotto boss and his collisions are terrible
-Trial of Chaos - when accompanying the Statue to it's location, sometimes the monsters keep spawning indefinitely

High delay, especially in 'cities'
-80Mbps cable internet, connecting to close servers, still input delays in towns and populated places
