I had that bug too. I went to Kabala, Constrictor Queen and got them there xd. Maybe it will help u Guys

Since I didn't do the Chaos Trial in Cruel Act 3, I couldn't "progress" this mission line any further. Then it switched back to map not completed, also after I found it again, went there, the game started the quest line to kill the Bog Witch.
