Unable to repeat Act 3 Temple of Chaos

I'm unable to repeat the temple of chaos ascendency #2 in act 3. The ultimatum item you get to open the portal was gone after I failed the trial the first time.

I hit "close" and I can't re-open the trial now because the item is gone. There's no way to get it back from what I can tell.

Please help.
