"DX12 can't find texture" crash

I have had a crash a few times when multiple skill effects are on-screen at once. When the crash occurs, an error message appears saying "DX12 can't find texture".

This issue occurs most often when I am playing the Witch.

I had this just now while looking through quests on the map:

As of today I've gotten this error 3 times now. Did get this error on the 6th or 7th. All 3 times I've been on the gem support menu mousing over my options.

Happened on my monk first then on my ranger the last 2 times

I've been getting this exact error since launch of Early access, it happens every 2-3 hours.

I just had this bug also during Ascension Trials 2 rooms before 3rd boss!

Cry.

I can't progress any of my trials because of this crash. It happens every time I'm in the ultimatum, and then randomly at other times in the game.

This has been an issue for me since day 1.
