FPS drop into client crash on moderately juiced content (Delirium, Breach, etc)

The problem: When running moderately juiced content I get stutter, when I get a large amount of mobs on the screen FPS quickly drops to 0 followed by the client crashing to desktop. This is most apparent on Delirium and Breach, but also occurs on Harbinger / other juiced content. I've also had some audio crackle, not sure if it is related.

My specs:

Processor AMD Ryzen 7 5700G with Radeon Graphics 3.80 GHz
Installed RAM 16.0 GB (15.3 GB usable)
System type 64-bit operating system, x64-based processor
Video Nvidia GeForce RTX 3060

What I've tried so far:

-Updated Nvidia graphics drivers
-Dynamic Culling = enabled
-Target framerate = 60
-Lighting = low
-Networking mode = Predictive
-production_config = Set most sounds to false, including sound effects
-Nvidia Control Panel = Manage 3D settings, shader cache size = unlimited

These changes have reduced but not eliminated the issue, and don't prevent 0 FPS crash if I push content. The PC exceeds recommended setting, but is running like a much lower strength machine.

I will post in-game metrics next while issue is occurring, I'm not fully confident in how to interpret them.

I can't capture a full on delirium / breach metric before the client crashes to desktop.
I should also add that I'm running POE on a SSD, same one that is running the operating system, 118GB free out of 476GB.

Renderer was set to DirectX12, but tried DX11 as well, no improvement.

Looks like CPU and GPU both hit between 50 and 150ms while the issue is occurring, I assume this is abnormal?
More updates as I continue to troubleshoot, none of these have helped so far:

-Set CPU power to 80%, no change
-Unchecked CPU 0, no change, PC reverted changes
-Lighting - shadows --> shadows + global illumination
-Texture quality - High --> Medium
-Remove all MTX
-Nvidia Power management mode - Normal --> Prefer maximum performance

Here is the state that replicates the issue the fastest, I realize this is more than 'moderate juice' but the weird thing is it starts before I open a single breach, just when I'm clearing native map mobs:

Map - Jungle valley or dunes
-Domination scarab of evolution
-Beyond scarab of invasion
-Breach x2
-Breach scarab of resonant cascade
-Breach on map device
-Blue altars

Before this I was running Harbinger on City Square and the problem was considerably less severe.
Further testing- tried switching to Dunes, and lowered the juice to this:

-4x breach scarab
-1x beyond scarab of the invasion

Still had the issue once I opened a breach and got some mobs going. This time I looked more closely at FPS- FPS drops down to single digits even before CPU / GPU hit 50ms. If I add more mobs FPS drops to near 0 and both GPU and CPU bars shoot up to 150ms+. If I push it even further it crashes to desktop.

Any / all help is appreciated, definitely shortens my league when I can't progress very far past maps. Thanks!

Update- Reverted all settings back to defaults as they make no difference one way or another, don't want to be testing too many settings at once. No PC issues on other more graphically intensive games.
Its just the game. U may have to lower settings.
