Viper Strike of the Mamba no poison being applied with Perfect Agony

So quite a lot of times I notice that I barely damage a mob but no poison is being applied. It isnt block, isnt map mods (e.g. poison avoidance). Im fully hit and crit capped. Could this be because I reached some sort of cap and poison stopped functioning? This is my character:

Bump; I am encountering the exact same issue.
It sounds like you're not critting? PoB assumes you are always using Ambush to attack and you didn't do something like Expedition cannot be crit modifier or some other crit avoidance. No crit = no poison = no damage.

Not seeing anything else immediately without any extra details. Plenty of people are using the combination right now so it sounds more user error to me. The overflow DoT cap bug was fixed awhile ago so I doubt that is relevant here. Would recommend video showing all modifiers and conditions that cause no damage.

Made further tests. I specced the Attack Mastery's "Mobs cant block node" yet the issue persists. I am 100% crit AND hit capped WITHOUT Ambush. This happens on normal white maps too without any mods.

Yea, I'm out of ideas on this one. Double checked your PoB and without Ambush and only crit flask it looks like you're crit capped, poison capped, have enough accuracy, nothing else causing poisons, and no weird modifiers. Re-watched a few parts to be sure the crit flask was up when nothing was happening.

It's possible PoB math is wrong for your crit chance if it never happens when Perfect Agony is off, but way out of my math skills to check that.

Yes, I watched it. I just caught up skimming other bug reports and you're definitely not alone on this issue. There were theories it was harby related, but now people are leaning towards an issue with Bino's? Which would line-up with the video if you have a poison value aura of 0, but the enemy is considered poisoned so they can't be re-poisoned until the aura runs out.

Would not match the working without Perfect Agony unless the test was not long enough to hit the Bino's bug. You could try additional tests with Bino's and no Agony and different weapon with Perfect Agony to confirm.
Hi! This can happen not only on harbinger monsters. I got the same lack of poison proliferation with monsters added to the map via scarabs, notably Monstrous Lineage and Adversaries. Prolif works correctly if said added mobs are mixed with other monsters, but on their own refuse to prolif just like harbinger ones.

UPD: forgot to mention that I don't use Bino's, just rare claws/daggers.

It isnt Prolif issue for me. THe monster I hit does not get poisoned. I am stopping the league, this is way too frustrating. I am starting to pull my hair out. Sometimes I have to hit a mob/boss 4 times to get poisoned which is IMPOSSIBLE because my offhand has 100% crit chance (and ofc 100% hit chance, I know about the double hit check for crits). In the case I ambush I have 100% crit chance wwith both of my hands.
