So max damage only on tanky monsters (for 10 seconds) after 5 hits and if it dies the blood charge is 0 again.

This mean you're locked in place for 5 casts each time it resets. Seems clunky AF when reading the gem itself.

The best way to make it smooth (or bearable) would be to combine it with unleash.

In skill description it says that you "Lose a charge when enemy affected by debuff dies" so if you kill a tanky monster you will lose 1 charge, not all of them. Also, charges are gained if you hit enemies and none of them die so investing in hit damage is not a good idea. You have 2 choices:
- have very low hit damage but fast cast speed to stack few Blood charges and high DoT before pack dies
- have very high hit damage and disregard Blood charges for clearing
Blood charges are still effective on bosses in both cases. This is to make skill not too OP for map clear but still strong vs bosses. And it scales with content difficulty - if on regular maps mobs are dying after 1-2 hits on t16 full deli you will have more time to stack charges on a pack of monsters. Basically skill "adapts" to enemy difficulty getting stronger the stronger the enemy is, which is cool.

I somehow read it as losing all charges.
This makes maintaining the blood charges less annoying.
