Delirium in act 5 Chamber of Innocence

Not sure if this is a bug, but I remember in one interview with Chris Wilson him saying that Delirium mirror should first appear after killing Innocence, which makes sense, because story wise there is an additional lore there with the mysterious voice. As you can see though while leveling my second character I encountered Delirium mirror when I entered Chambers of Innocence for the first time.
