Has there been a general "fix"?
I've been getting these lags since early Legion League (not at the start, tho).
During peak hours from 6pm to 11pm in Europe most instances I create (including hideout, alva temple, safehouse, literally anywhere) will have lag spikes from 40ms to 2000ms, making those unplayable (next map could be 100% fine). This is really annoying when playing fully juiced T16 maps and I literally cannot run those because of the risk of dying on lvl 95+
Also, if I want to trade with other players those other players are joining my hideout in a different instance than I am in. Resulting in me having to refresh my hideout instance and reinvite my trading partner.
WinMTR Reports:
fra.speedtest.pathofexile.com - https://pastebin.com/vwx3Sy29
fra-m.speedtest.pathofexile.com - https://pastebin.com/64ZPAY1h
fra.login.pathofexile.com - https://pastebin.com/sP2s377k
ISP is Deutsche Telekom like for most others in this thread. Calling them resulted in them telling me that their network is the "best" in Germany and even in peak hours there are no issues.
Hoping for a fix soon - lvl 100 with those lags this league is impossible. :(
Since you are getting instances without lag, my advice to you would be that you start a local log where you register the instance IP's with LAG and the instance IP's without LAG.
Then have GGG explain why this is happening.....

Well the new ISP's they are getting around in the world is pretty new and wasn't there in Synthesis -->
fra.speedtest.pathofexile.com - https://pastebin.com/vwx3Sy29
fra-m.speedtest.pathofexile.com - https://pastebin.com/64ZPAY1h
You can clearly see the routes are not the same.
That's why I am guessing that if you do the log, you will see that the bad instance IP's belongs to the one ISP and the good instance IP's will belong to the other ISP.....

i had a ping of around 160ms. i loaded my vpn and got 50ms. i turned off my vpn, and it went back to 160ms. loaded vpn again, back to 50ms...
does this seem like a net neutrality issue?
is there a way i can solve this without using a vpn service?

This is because when you use the VPN, the route to GGG hosting ISP is differant and you no longer use the piring router that is the bottleneck from when you are not using a VPN.

nearly unplayable again. I noticed it happens most of the time in the afternoon. When I'm online late at night I haven't got these problems. Was doing glacier maps and every few my ping went nuts. Client.txt shows this:
2019/08/15 19:34:51 2239175 fd [INFO Client 5312] Connecting to instance server at 185.225.208.206:6112

2019/08/15 19:36:38 2346114 1b9 [DEBUG Client 5312] Connect time to instance server was 62ms

2019/08/15 19:38:33 2460697 1b9 [DEBUG Client 5312] Connect time to instance server was 1061ms
after that I got another instance which was fine again:
2019/08/15 19:39:40 2528261 fd [INFO Client 5312] Connecting to instance server at 185.225.208.206:6112
2019/08/15 19:39:40 2528277 1b9 [DEBUG Client 5312] Connect time to instance server was 16ms
I can reset just by using a portal in and out of the instance. Heres the winmtr: https://pastebin.com/Xv7U2PcW

Still awful connections in the evening - currently unplayable. No use playing Blight if you don't get that fixed.

Bump, issue pesists, unplayable even when using a (ger/nl/uk) VPN.
Will likely get worse with blight.

Which VPN company did you use?
I am a customer of NordVPN, they have a 30-day money-back guarantee, had no problems with them.
Try using one of their different EU countries ?connections?, that could reasonable not be hanging in on Telekoms ?infrastructure/peering?.
They also offer a dedicated IP service[1] with their standard package and Double VPN service.
