Not sure if anyone else has mentioned this yet, but I had a T2 room not working properly, I guess I left my unique item in the table and on my next Temple I had another T2 sacrifice room and my unique item from the first temple was still in the table of sacrifice.

Same bug for me. The Temple of Atzoatl with monster lvl 75.
Room with Table of Sacrifice LVL 2.

Tried to change uniques:
weapon = apep's rage
belt = perandus blazon
helmet = hrimnor's resolve
amulet = Araku Tiki
jewel = Tempest Flesh

Nothing happend every single time, unique items didn't change.
Every time game wrote "Crafting failed: The operation could not be completed"

Just had this happen. Death's Harp, level 1 table, monster level 63.

Same thing here. Tried Kikazaru, Fairgrave tricorne and some dumb boots.

Nothing. :(

Have the same issue. Tried a total of 12 uniques and it still didn't work :(
