just ripped in Flashback HC, not sure how i died

lvl 91 Gladiator, was in a T12 Overgrown Shrine, killed a harbinger pack, soon as they died the entire pack exploded or something; insta-KO'ed. there were no tormented spirits and no totems on the map and it wasnt beyond. but...i didnt play Harbinger so is there something i just wasnt aware of?

91 aint bad, Try 99.85 like AilaTWPlayer. He had win@100 SSF HC wrapped up fr 5th time in a row but blue screened.

Maybe one of the Bloodline packs spawned a Beyond monster.
Even if the map itself does not have the Beyond mod some of the individual packs (usually the last before the boss) can spawn one of these these. Could be Bameth in your case: https://pathofexile.gamepedia.com/Bameth,_Shifting_Darkness Masterpiece of 3.16 lore
