For everyone mentioning about this free box not showing up for them in their game, here's why:

1) Firstly I believe you have to have your forum account here on the site linked to your steam account, otherwise GGG will have no way of knowing where to send the free MTX box.

2) Also please check the "Account Transactions" section of your profile/account here on the site, and make sure the free MTX box shows up in there. If it does not show there, then see #3 below.

3) There was apparently only a limited number of free MTX boxe's that GGG allocated for this holiday gift event. If you have done both things above (#1 & #2) and still do not have the MTX gift box in-game. Then sorry, unfortunately you have missed out on this MXT offer, and will not be able to redeem it.

I got a Ice Shield, which is rather nice, since I have been collecting Shield MTXs since this summer, so its something I will actually use at some point.
