Complaint about WB - something about (client?) lagginess triggering the misbehavior: https://www.pathofexile.com/forum/view-thread/2019855

That's how the game works. Mouse pointer position isn't synchronized nor immediately registered when mouse moved.

0.75 attack time WB works fine at 40ms latency and start zipping randomly at 90ms+
0.45 attack time WB (necro-support in party) doesn't work properly even at 20ms.

With high attack speed and high latency you starting next WB before the game synchronizing 'new' character-relative mouse position.

So basically you need stable 120+ fps and 5ms latency to never face that issue.

same problem. i thought it was just my latency and what you explained basically matches my guess. thanks for sharing.

I'm pretty sure there is a conflict with the mouse click movement, in fact this does not happen if I leave the buttons free and spam WB

I have seen at least one claim stating that the client does not correctly sync mouse (not sure if position or buttons or both) with the game state. So it may well be a cause for this/these problem/s.

Yeah, that's been annoying me tremendously, too.
Use movement skill to get out of troubled area, turn around and attack: Boom, back in trouble.

It's not just movement skills, though. The input simply gets handled badly if you switch between 2 skills very quickly. I use a totem, a curse, then my attack. It is depressing how often I get two totwms and my attack instead of that curse. Really lousy programming here. Oh, and it's a mix of mouse buttons and keyboard in my case.
