It is time to admit the obvious. I have been slacking, and generally not punctual with the maintenance of this thread. It is now time to update the order of the imbedded items, and change the majority of the supporting text.
The changes implemented in 3.14 with Ultimatum league (global drop pool), and the upcoming changes in 3.17 (Prophecy removal) make much of this current structure outdated. Additionally, with the existence of the Unique Collection Tab, there has been more enthusiasm for collecting Unique Items and a standardized display order. I will be working on changing the display order to match the UCT order (even if some of the order decisions make no sense from an cataloguing perspective). I will also review and trim the supporting text from the front page of the thread.
For posterity, the six posts that make up the bulk of this information will be preserved in the spoilers following this post (also because finding and transferring imbedded items is a pain sometimes).
The unique items added in patch 3.12.X will be added here as they are spoiled or found. New stuff will always be at the top.
This thread provides a list of every unique item in Path of Exile, and a place to consolidate sightings and discussion of newly added uniques. If you find or see a new unique that is not on the list and would like to contribute, please post the item here, without any socketed gems. If you are unsure how to post an item, click the character portrait at the top left of the page while posting a reply. This will open your inventory and stash, and clicking items in this window will insert them into your post. Screenshots are also accepted as a temporary measure for brand new items. If you have a theory about an upcoming unique item and can support it with evidence, feel free to post it for potential inclusion in the speculation section.
The Fifth post contains more general information on uniques and how to obtain them. Much of this information, as well as further information on item mechanics and assorted other topics that are beyond the scope of this thread, is also available at the Path Of Exile Wiki.

World Drops — Most unique items can be found as random drops from monsters, chests and breakables just the same as any other item. Normal monsters drop items with an item level equal to the zone level, Magic monsters drop items with an item level equal to the zone level + 1, Rare/Unique monsters drop items with an item level equal to the zone level + 2.
For example: If a unique item requires lvl 50 to equip, that means it can be dropped by all monsters in lvl 50 zones or higher, dropped by Magic monsters and above in lvl 49 zones, and dropped by Rare or Unique monsters in lvl 48 zones.
A small number of unique items have item level requirements to drop that are substantially higher than their level requirement.
Drop Rate — The actual drop rates of unique items are unknown to us. However, some information on their mechanics has been shared by GGG or determined by the community. To the best of our understanding, there are four discrete tiers of unique item rarity. See this thread for the community's best guesses of which items belong to each rarity tier. Every unique within a tier has an equal chance to drop. Exceptions to this tier system are unique jewels (which have three separate tiers, all rarer than the most common tier of non-jewel unique but otherwise mysterious), and tier 1 and 2 league-specific uniques (which are approximately four times rarer than their non-league-specific counterparts).
Divination Cards — Many divination card sets can be redeemed for specific unique items or a random unique from a certain subset. Refer to this wiki page for more information on divination cards in general or on specific cards.
Orb of Chance — There is a very small chance of upgrading a Normal item into a unique of the same base type when using an Orb of Chance. For example if you want the Shavronne's Wrappings unique body armour then you should use an Orb of Chance on an Occultist's Vestments of Normal rarity. Exact success rates for Orbs of Chance are unknown, but appear to correlate with the rarity tier of the item (see "Rarity Tiers" above).
Boss Drops — Certain unique items drop only from specific unique monsters. Some of these items can also be obtained through divination cards, but they cannot be chanced.
The Pale Council fight has a guaranteed unique drop based on the last member of the Council defeated. The last Council member left alive is random, and in version 3.0 the unique items were changed. Now you find Eber's Unification from Eber, Inya's Epiphany from Inya, Volkuur's Guidance from Volkuur, Yriel's Fostering from Yriel. The original unique items from The Pale Council are still available. Now they have a chance of dropping from the final step in the prophecy chain for the individual Pale Council members. Breath of the Council can drop from Eber, Grip of the Council can drop from Volkuur, Mind of the Council can drop from Inya, and Reach of the Council can drop from Yriel.
The Guardians of the Void each have two exclusive drops. Note that these are not guaranteed, although the chance of receiving one is quite high compared to normal unique items.
The Breachlords and breach monsters from their domains have the ability to drop exclusive unique items. The Breachlords have an additional chance to drop a 'Blessing' which can be used to upgrade their unique items. Note the unique items and BLessings are not guaranteed.
The Elder Guardians each guardian has at least one exclusive drop. Note that these are not guaranteed, although the chance of receiving one is quite high compared to normal unique items.
The Spirit Beasts are considered the final bosses for the Bestiary league mechanic. There are four Spirit Beasts and each has a set of four unique items it can drop. Each encounter with a Spirit Beast will guarentee a unique item from the beast's drop pool.
The Delve Bosses, found within rare city biome encounters, are the new bosses for the Azurite Mine. These bosses can spawn at a minimum depth of 83, 150, and 250 respectively. Introduced in the Delve Challenge League, the three Delve Bosses each have their own pool of unique items. Each encounter with a Delve boss will guarantee a unique item from the bosses drop pool.
Vendor Recipes — Some unique items can be obtained only via vendor recipes. See "Vendor Recipes" below.
Corruption — Some unique items can only be obtained by corrupting other items, and therefore only exist in a corrupted state.
Obtaining League-Specific Unique Items — Some unique items were designed specifically for one of Path of Exile's regular temporary leagues, and were only able to drop normally in that league (or in a pair of concurrent hardcore and softcore leagues). These items can still be obtained by using Zana's Map Device to add the appropriate league's property to a map. This will allow uniques specific to that league to drop or be created via Orbs of Chance while within the map. Note that the same league-specific mods will not grant this property if obtained from any source other than the Map Device (for example, The Harvest will not drop in a map which has the Beyond affix, or in an event with the Beyond property, unless the above process is also followed). Alternatively, certain divination cards can grant league-specific unique items. Some league-specific unique items (e.g. Talismans, Perandus Manor) will not drop randomly even in the appropriate Zana-modded map, and must be obtained via whatever process was originally used to do so.
Race Rewards — Each race season introduces a set of unique items with alternate 2D (and sometimes 3D) art, obtainable only for achieving a certain number of points within that race season. See this page for information on the current season, and refer to the race rewards section above for a list of past race prizes.
Event Prizes — Occasionally, unique items are offered as top tier prizes in special event competitions. Please note that not all events offer unique items as prizes.

Recipe: 1 x The Goddess Bound (20% quality), 1 x Elegant Sword (20% quality and sockets must exactly match those of The Goddess Bound [same number, link and color]), 1 x Orb of Fusing, 1 x Chance to Ignite (at least 1% quality)

Oni-Goroshi has replaced The Goddess Unleashed in this series of swords. It can only be obtained from Hillock in the Twilight Strand.

Some unique items in game have the ability to roll with one or more mods from a pool of selected mods. This resource will not be providing a full list of all the options for items with variations, but instead list the items with a general sense of the variants.
Poorjoy's Asylum - This map has the mod "Unique Boss drops (10 to 15) additional Rare <Item Class>." <Item Class> can be one of 22 different base item types.
Doryani's Invitation - This Atziri belt can drop with a focus on one of four damage types; Physical, Lightning, Cold, or Fire.
Atziri's Splendour - This Uber Atziri chest armor can drop in nine variants, each focusing on a different, or multiple, defenses; Armour, Energy Shield, Evasion, Armour/Energy Shield(with Max Life), Armour/Energy Shield(with Max ES), Armour/Evasion, Energy Shield/Evasion(with Max Life), Energy Shield/Evasion(with Max ES), or Armour/Energy Shield/Evasion.
Vessel of Vinktar - This flask can spawn with one of four damage modifiers; added lightning damage to spells, added lightning damage to attacks, lightning damage penetrates resistances, or a portion of your physical damage is converted to lightning damage.
Skin of the Lords - This upgraded Breach Chest has one random keystone from the passive tree, excluding Chaos Inoculation and Necromantic Aegis.
Volkuur's Guidance - This pair of gloves from the Pale Court can drop focused on one of the three elements; Lightning, Cold or Fire.
Yriel's Fostering - This chest piece from the Pale Court can spawn with the focus on one of three de-buffs; Bleed(with Drop Bears), Poison(with Snakes), or Maim(with Rhoas).
Unique Items with Abyssal Sockets - Unique items with Abyssal sockets can drop with either one or two Abyssal sockets. Exceptions to this are Darkness Enthroned which only drops with two, and Shroud of the Lightless which now only drops with one.
Impresence - This Elder Amulet drops with a focus on one of five damage types and the type's associated aura; Physical(Vulnerability), Lightning(Conductivity), Cold(Frostbite), Fire(Flammability), or Chaos(Despair).
Watcher's Eye - The Elder jewels dropped during the elder encounter will always have two aura modifiers while jewels dropped during the uber elder encounter will always have three aura modifiers. The total pool of aura modifiers is 71. Watcher's Eye takes the prize for total number of variants in the game with a total of 2,485(two mod), or 57,155(three mod) different possibilities. See the link for a list of the modifiers.
Shadowstich - This corrupted Sacrificial Garb, an outcome of the 'Locus of Corruption,' is a six linked chest guaranteed with two of the chest corrupted implicit mods.
String of Servitude - This unique belt only drops from the Vaal Omnitect in the Temple of Atzoatl. It drops with any one of the belt corrupted implicit mods.
Aul's Uprising - This unique Amulet only drops from Aul, the Crystal King. It drops with a focus on all or one of the attribute stats. The all attributes version provides the Envy skill, and the modifier 'Envy reserves no mana.' The single attribute versions provide a 'reserves no mana' modifier to a random aura aligned with the attribute. Example: A dexterity based version might have Hatred reserves no mana, or a strength based version might have Vitality reserves no mana, etc.
Precursor's Emblem - This unique ring is obtained through a vendor recipe consisting of three Delve Component rings; one 'Meadow,' one 'Mountain,' and one 'Valley.' There are nine possible component rings; Three ruby rings 'Ahkeli's Meadow/Mountain/Valley' aligned with Endurance Charges, three sapphire rings 'Putembo's Meadow/Mountain/Valley' aligned with Power Charges, and three topaz rings 'Uzaza's Meadow/Mountain/Valley' aligned with Frenzy Charges. The rings used will dictate the base type of the resulting Precursor's Emblem, and influence the charge modifies. Example: All three ruby component rings will create a ruby Precursor's Emblem with three Endurance Charge modifiers, two sapphire and one topaz ring will create a two-stone(cold & Lightning) Precursor's Emblem with two Power Chargs mods and one Frenzy Charge mod, one of each will create a prismatic Precursor's Emblem with a mod for each type of charge, etc. For a full list of charge mods click here
Doryani's Delusion - This unique pair of boots only drops from The Apex Assembly in the Doryani's Machinarium unique map. During the map you will make choices to augment the boss three times. Each time choosing between fire, cold, or lightning. The buff to minions chooses base, claw chooses added damage, and bludgeon chooses the Purity.
Circle of Anguish, Fear, Guilt, Nostalgia, and Regret - These rings can only drop from Synthesis Memory Nexis Bosses. Each ring has one of five possible herald modifiers. After being synthesized the ring will have two of the five modifiers. The links provided have all the possible modifiers per ring.
Megalomanaic - This unique cluster jewel drops corrupted, and has three cluster jewel notables. (List of notables in the above link)
Split Personality - This unique jewel drops corrupted, and has two modifiers from the following list; Dex, Str, Int, Armour, Evasion, Energy Shield, Life, Mana, or Accuracy.
A legacy item is an item with stats different to the version of the item that can currently drop. Legacy items are a byproduct of balance changes to items, as items are coded in such a way that values of item mods cannot be easily changed en masse. Thus, retroactively changing values for all existing items would require literal days of complete server downtime while a script ran, which GGG considers to not be worth any benefit to permanent league balance it may cause.
Note also that changes to a unique item do not always cause existing versions to become legacy. Changes to the effect or mechanics of a mod on an item will be applied retroactively. Changes to the value of a mod, or the addition or removal of mods, will not be applied to existing items. On a similar note, a Divine Orb will reroll the values of a legacy item's mods to within their new range rather than the legacy range, but cannot add or remove mods to bring the item into line with newly dropped versions.
The only way to obtain a legacy item is to trade with another player.
For a complete list of legacy uniques items, refer to the Wiki. Aggressive rebalancing of certain stats has made keeping a list up to date manually too time-consuming to continue with.
A unique item, easily spotted by its Orange Text and orange banner, is one that always spawns with the same mods(sometimes with different values) on the same base item with the same requirements.
Unique items may have their mod values rerolled using Blessed or Divine Orbs. Some mods, however, have static values and cannot be changed.
Orbs of Scouring & Mirrors of Kalandra CANNOT be used on unique items.
Possessing a Keystone passive and equipping a unique item which grants the same Keystone ability will NOT double the effects of the ability - i.e. the effects do not stack. For example, a character that has allocated the 'Mind Over Matter' Keystone and is also equipped with the Cloak of Defiance unique body armour will still only redirect 30% of incoming life damage to mana, not 60%. Similarly, if you equip an item that has the mod 'Blood Magic' but you already have the 'Blood Magic' Keystone, the effects of the item mod will be wasted.
The item level (ilvl) of a unique does not affect its mods in any way, but does influence how many sockets can appear on it. While the stats may be the same, a unique with a high item level may have a higher selling price when trading with other players.
All mods that include 'when on low life/mana' are activated when the player's life or mana drops below 35%. Mods that include 'when on full life' require the player to have 100% of their life total. Life and mana reservation are included in these calculations. Note that a player with the Blood Magic keystone passive is considered to be neither on full nor low mana.
"Gems in this item are Supported by level X [gem]" means they are supported by a support gem of level X. If that gem has a mana multiplier, it is applied as normal. "Gems in this item have [property]" does not use a specific support gem, so no there is no mana multiplier.
"Grants level X [skill]" gives you a skill of the specified level to add to your hotkeys while wearing the item, exactly as if you had socketed a gem. It does not passively grant any bonus (e.g. an item that grants an aura skill does not cause you to be permanently affected by that aura — you must activate it and reserve mana as normal). Skills granted in this way are automatically supported by all applicable support gems socketed in the item which grants the skill, regardless of links. Such skills are not associated with a gem and thus cannot be supported by Empower, Enhance or Enlighten.
"Nearby" is a word that appears on a few unique items, as well as other effects. The radius of these effects is not standardised, although the radius of certain "nearby" effects may have been determined. Checking the Wiki is advised.
