If you dont mind dropping Carcass Jack (i dont no need for more dps if you subtract Uber) - use Geofri's_Sanctuary. ( if you dont mind losing 3 points - Carcass is still ok)

Rest is self explanatory. Also dropped Void Manipulation and replaced it with Increased AOE/Concentrated Effect ( i know poison doesnt scale with it, but 20% more initial hit makes up for it)

I can reserve all mana so running Enfeeble also helps.

But to be honest you can drop anger for curses. I dont have +1 neck so ill pass for now. Enfeeble is just too good to not run it.

Survivability seems to be the problem in OP's build, but some tweaks can fix it :)
