If an item is lvl 68 (as per the Armour Data Table) , e.g., Kaom's Heart, it requires at least character level 68 to equip. It also requires at least item level 68 to begin dropping. In order to get at least item level 68, see the post you quoted.

Another example: Quill Rain requires character level 5 to equip. It also requires at least an item level of 5 to begin dropping.

There are few exceptions to this required level = minimum item level coincidence. Those exceptions would be the low level items that do not have attribute requirements, and in some cases items obtained via vendor recipes.

For the cloak of flame, does someone know if the damage conversion happens before or after armour being factored?

And if it's before, does it then mean that only 80% of the hit is being factored with armour?
