Except that 'life on hit' is not the wording that is actually used in-game. On every source of 'life on hit', what you actually see is '+X Life gained for each enemy hit by your Attacks'. That's pretty clear once you know the difference between Attacks and Spells. (The only possible ambiguity there is what happens if you hit the same enemy multiple times with the same Attack.)

Although sometimes they use "Physical Attack Damage" and other times just "Attacks", suggesting one only affecting Physical, i.e. the other affecting both. And although it says "Attacks", it's named "Hit" both the gem but also in the Offence Tab.

Either way, it's definitely not clear, why we're having this discussion in the first place. If you're leeching life out of your attacks damage, enemies killed or number of attacks shoudn't make much of a difference. It's still leeching out of something, and it definitely gets ambiguous with a talent like Ghost Reaver. Why you in a game where it's so hard to respec more than a few points - in most cases easier to make an entirely new character and spend a large amount of time playing it to the level you already reached - you want to make it crystal clear so there's absolutely no doubt, adding some sort of extra in-game description if there's the slightest doubt, otherwise you're only giving newer and more casual players a quite rough time in the long run :/
