Trial of Choas soul core bug

My brother and I placed our soul cores into the altars and it did the animation but when trying to pick up the third and last soul core it said we already had one..... then the altars were empty and when clicking on them it said we had no soul cores..... then we died from holding the soul cores too long.. I have a clip of it saying we had one and we dont have one
