Witch Demon Form bugged after visiting act 3 camp.

Witch Demon Form always disappearing when I use ANY another skill. It started after I enter act 3 camp zone and my DF was still on me somehow. I tried to respec it/remove gear and skill/support gems etc.. Even after latest patch this problem still exists for me.
