Phaaryl Megalith rewards unable to open

Not sure what caused it entirely but I was unable to open all but 2 rewards after completing all boss waves. I think it was due to the last "wave" with double spawns being Balbala and another phasing boss. Balbala's coin kept resummoning her "spirit" after she was dead. Yes, I did check the entire map in case it was a fog thing and killed the spirit about 10 times and even portalled out and back in killed spirit a few more times, nothing.
