MS spikes and 30 second game freezes constantly after patch

My game freeze for 10-30 seconds constantly, MS and CPU show hughe spike to 1000+, if i pause the game it dissapear but if i unpause it instantly comes back. Slight chance it has something to do with whirlwind spear as im constantly throwing them out and jumping in and out of them to explode with rake / dodge / dissengage when it happens. There was 0 issues for me before 0.2 but now my game is unplayable. I have tried every trick i can on my end, reinstalling drivers, clearing shaders, increasing shader size, changing to vulkan, changing graphics etc but it makes 0 differance.
