How to fix breaches being bricked by map layout
Make all breach encounters just like the Abyss areas in PoE 1.
Any map with the breach mechanic should have a separate part that is empty (you can add cool assets kind of like the Belly of the Beast areas in PoE 1). Alternatively, make it have one of 3-5 specific layouts that are all balanced among themselves (no layout is better than the other) Once you interact with a Breach hand, it'll do some cool animation and open the breach portal. Entering the breach teleports you to the center of that separate part of the map and it instantly starts the breach encounter (the expanding circle). Once the breach encounter ends, the circle collapses towards the center and does another cool animation into becoming a portal back to "reality". A map with multiple breaches has multiple separate areas. Pros: Make breach encounters balanced across all map layouts (because they don't rely on the map layout for anything anymore) Prevents breach encounters from severely cluttering the rest of the map with items and makes it easier for you to spot and collect any breach valuables because there's no loot from the map either Cons: You can't open a breach and clear the map at the same time, which will cost some time from efficient players Maybe break the breach lore because they live in another dimension "under/over" our own, not separate?
