Trial of Chaos

While completing Trial of Chaos on the Stones Trial the 3 stones did not spawn in any location. When interacting with the middle platform the graphic of a stone appeared on my character and the message said missing reagent.
The only way to fix was to die. I couldn't use any form of exiting via traversing back through the stage or town portal. I had to force death loose xp and reset outside losing map stone.
