Boss spawns in location I can't access

I was doing an Atlas map with a waystone, and the boss and two alters spawned in a place I could not get to. Here's a couple of screenshots showing the map and boss. You may need to make the image full size, but you can see the last boss in the middle of the map in an area that's not accessible.
