2 unobtainable skill points

I currently have 22 weapon skill points (should be 24)

I have ran and re-ran every weapon skill giving encounter in the game several times now, I can't find anything missing. I suspect it got bugged when I had a duplicate quest item from normal and cruel (possibly blood sacrifice) it's gotta be a drop item that ain't dropping anymore.
