Sorceress - Instant death

For some time now, something has been killing me on maps, and I have no idea what it is. You can see it in the video at the 40th second. I have full shield, full mana, and I get instant death. I've talked to several players, and they have the same problem. Is this a bug? I also play a monk with 'Chaos Inoculation', and I don't have this issue. What's going on?!"

My guess is you have "remove life" effect from somewhere, I personally have not mapping much so I can't say for sure if you have a nasty map mod or rare mob... but... even though very unlikely, does your Mana flask have "Remove 15% of recovered mana from Life"?

On the mana bottle, I have "increased amount recovered and charges gained". I'm trying to figure it out, but I don't know what it could be. I had this problem on 3 maps in a row.

Could you post your gear and tree? I notice you die as soon as the ignite debuff pop up on your screen, if there is any unique effect that make ignite deal damage as though Bleeding or "% of damage overtime bypass energy shield" it would be the issue.

It is most likely the helmet is at fault here.
Consider you have CI, that helmet's unique effect provide no benefit, even if it's not the cause of the problem I would personally switch it out for a high ES base rare helmet.
If the helmet still kill you with Mind Over Matter allocated then it would be a bug, but with your posted tree (without MoM), it make sense that you die because of that helmet.

Yes, it was the helmet's fault. I changed it and stopped dying :)
