First time i encountered that in many many many maps, and obviously had to happen on the best map ever Vaal Factory (no sarcasm here)

No idea if it was spawned in like this or how it happened tbh.

had that same thing happen also in vaal factory
rare was stuck under floor, couldnt damage it with any aoe, didnt prevent the rare from shooting me however
map was also not completable
