Dying but not Dying

this was never happening before the large patch after christmas but now i keep dying somehow just like this while still overcapped on shield and full mana with mind over matter and i dont lose xp when this happens no resurrect button and still can move around and my heralds go off and i can still die if i go lose me energy shield and mana while crawling around this is happening almost every map now not sure what to do or if this is even the right place to post this
