Can't Proceed in Chaos Trial

I died on the boss during the first chaos trial, now can't re-attempt it.

The door is open, but when I click to enter it says something like "the instance no longer exists", but when I re-enter the subscribed ultimatum to open a new instance it says "the operation could not be completed".
