Stalker spirit in temple of chaos

The ghost/stalker/spirit that is supposed to follow you and the run is over in 7 hits doesn't seem to work properly, sometimes it does not appear at all and sometimes it appears randomly then gets stuck somewhere. And some runs it has worked 100%, It leans more on the side of not spawning at all my last 10 runs or so in temple of chaos.

Obviously after choosing the debuff, do not mean I. doesn't appear as a choice with the taskmaster. Just to clarify.
