Omen of Amelioration failed to activate on death

While running a simulacrum died in the last wave and the Amelioration failed to activate, i had 2 omens enabled amelioration and resurgence the resurgence activated but the Amelioration did not.

If you read the omen description, only one can be activated at one time (instance). Well, you can activate both, but only one will work (be used).
Game randomly chose one of the two to use. |
