I have beat king of mist on several characters and not gotten the spirit tried several times

I am unable to get the first base spirit addition for several characters and need help I have tried new instances and rerun several times. Please advise

The quest item (green) that he drops has to be right clicked to get the spirit, or are you saying he's not dropping that?

Yes the quest item is not dropping it is on 3 of 7 characters I have going
