Trial of Chaos - After failed run cannot open another trial - Cannot complete operation

The bug happens when the character dies on trials that allow for ascendancy points to be earned late game (the non reusable ones level 79), once kicked out to the arena entry when trying to start a new trial on the altar the game responds with a "cannot complete operation" message . Instance needs to be reset in order to run another trial (tests do not indicate difference between common and ascendancy ultimatums - same error message happens for both) .
